As I said in the last installment, we are an 'alter' in Elliot's system. So what does that do to us? Even though we don't learn that specifically until the end, we are told by him that we are his imaginary friend. He calls us this incorrectly because that's what he believes at that time. Alters aren't imaginary but since Elliot is unaware of his own disorder he doesn't have the right vocabulary to describe us. He chooses to create us and calls us his friend. His confidant really. The show starting with the line "hello friend" is really just one of the best ways to introduce us to Elliot.
